A team is always picking reasonable action items but is later not delivering on them. How do you handle this habit?
Is it related to Retrospective action items? If so, I will see how many they are taking up. If they are taking up many, obviously the focus and commitment will be poor. So in next retrospective, I shall use DOT voting technique and help them to prioritize top 2 action items and I will use Fist of Five to check their commitment levels to achieve those selected items. I will also help from my side during the next Sprint to work up on those action items by reminding and get required support as and when needed.
